MARYLAND BOARD OF PHARMACY
RENEWAL APPLICATIONS and INSTRUCTIONS

This page contains only renewal applications and instructions in Microsoft Word format.   These download files are only available during the renewal period October - December 31st of each year. After the permits expire, you must contact the Board for reinstatement instructions.

The Board changes the status of all establishment permits not received in the Board's Office by the close of  business December 31st.  A computer program automatically runs after midnight, January 1st, changing the status from "Active" to "Non Renewed".   Expired permits may be subject to a reinstate process. 

Maryland Medicaid will not pay claims submitted by pharmacies with expired permits.  Maryland Medicaid Administration   Provider Services  (410) 767-5340
